Who are we looking for?
We are looking for a skilled and passionate customer-focused facilitator to join our Home Care team, providing support and service coordination to our valued consumers. Acting as the first point of contact for customers and staff, you will provide prompt and responsive service.
What do you need to bring?
- Certificate III or IV in Coordination or other health-related qualification (desirable)
- Outstanding customer service skills with a high level of communication and organisational skills
- Ability to proactively respond to concerns of consumer’s health and well-being
- Knowledge of issues supporting and caring for older people in the community and the ageing process
- Excellent organisation and time management skills
What can you expect to be doing?
- Organising, monitoring and reviewing basic care services with a focus on maximising the well-being and independence of our customers
- Acting as a point of contact for customers, their representatives and care staff for changes in circumstances and service requirements
- Work collaboratively as part of a team in a manner that promotes the best outcomes for customers
- Actively participate and contribute to quality improvement initiatives and programs
